RIGHTS AND RESPONSIBILITIES FOR HEALTHCARE SERVICES 

THE AHF PROMISE AND MISSION

To improve the quality of life for people living with bleeding disorders and to ease the burdens and worries of managing these chronic illnesses by offering each client a personal and specialized home infusion support program, including next-day home delivery of physician prescribed clotting medications and round the clock client services support. AHF is more than a business; it is the culmination of a lifetime of commitment on the part of our family, to people with bleeding disorders.

We believe that clients have:

v      The right to be treated with respect and courtesy within a setting that provides the highest degree of privacy and security of the client and client health information.

v      The right to expect their health related information to be confidential and secure as outlined in the AHF Notice of Information Practices*. 

v      The right to freedom from discrimination because of age, ethnicity, gender, disability, religion, sexual orientation, values, beliefs, marital status, medical condition, or any other arbitrary criteria. 

v      The right to have full access to information from the health-care provider about current FDA-approved or other proven treatments. 

v      The right to know the identities, titles, specialties, and affiliations of the care coordinators and all healthcare providers and about the rules and regulations of health care centers and organizations. 

v      The right to have information shared in a way that is easy to understand, taking into account differences in background, cultures, and languages. 

v      The right to be involved in and make decisions about the plans of care prior to the start of and during the course of treatment. 

v      The right to decline to follow treatment plans, trials, counseling, or any other services, as allowed by law, based upon the consumer’s judgment of risk and benefit, and without pressure or unwanted influence from the health-care provider. 

v      The right to be informed of all known risks associated with declining to follow treatment plans, counseling, and any other service. 

v      The right to name an advocate such as a family member or other person to support health related decisions. 

v      The right to have an advance directive, such as a living will, health care proxy, or durable power of attorney for health care.  And the right to have that directive followed within the context of existing law. 

v      The right to be kept informed of financial responsibility including the right to inspect and receive an explanation of health care bills or proposed charges regardless of payment source.  

v      The right to voice complaints and suggest changes and to be informed of the processes involved in voicing complaints and suggestions and the assurance that this right can be exercised without fear of interference, pressure or reprisal. 

v      The right to confidentiality and the right to expect health-related records and communications to be kept private and secure and the right to have access to all treatment records and communications. 

v      The right to adequate pain management, although the direct management of pain is outside the mission of AHF. 

Clients are expected: 

v      To treat staff and other medical providers with courtesy and respect. 

v      To provide correct and complete information about his or her health status, other treatments being used, and all other medications and natural remedies being taken. 

v      Take responsibility for the results of their decision to decline medical advice or treatment plans. 

v      Take financial responsibility for their medications, care, and treatments. 

Under Health Information Portability and Accountability Act (HIPAA)*: 

·         Clients have more control over their health information

·         Boundaries are set on the use and release of health records

·         Appropriate safeguards have been established to protect the privacy of health information

·         All staff have the responsibility to protect clients’ confidentiality

·         Diagnostic information and procedures’ schedules shall not be visible to others

·         Patient care documents are destroyed appropriately

·         Computer databases are protected and are accessible only to those who need that information (for treatment, care, and reimbursement services)

·         Release of records will be based upon authorization where required by law

·         Violators will be held accountable
